C. Scott Ananian 47545a5d6f Remove no-insertion metadata corner case from ve.dm.Transaction.pushReplace().
This version pushes a `replaceMetadata` operation after a `replace` to
fixup trailing metadata if there is no inserted region and the removed
region includes metadata.  This avoids a corner case where the
size of the metadata arrays inserted/removed in `Transaction.pushReplace()`
do not match the size of the data arrays inserted/removed.

We remove the now-unused `Document.getMetadataReplace()` method.

We also adjust `MetaList.onTransact()` so that it continues to work
properly when the number of metadata entries in `replace.insert` is
not the same as the number of metadata entries in `replace.remove`.

C. Scott Ananian 5830bce72c Correctly preserve metadata in Transaction.newFromUnwrap.
The `Transaction.pushReplace` method has a corner case if the removed
region has metadata and the inserted region is empty.  This works fine
unless there are two adjacent `pushReplace` operations, which can occur
in `Transaction.newFromUnwrap`.  Fix this by having `pushReplace` look
at a preceding replace and correctly merge the two operations if
possible (in particular in the tricky case where the previous case has
a zero-length insertion).  Pleasantly, this can be done without a lot of
special-casing code in `pushReplace` or `newFromUnwrap`.

Add test cases verifying the `newFromUnwrap` works correctly (both
in commit and in rollback) when there is metadata present.

C. Scott Ananian 5095e27925 Collapse metadata on any removal.
In ve.dm.Document.getMetadataReplace(), we used to only merge metadata
if the amount removed is larger than the amount inserted.  But this
could end up putting metadata in odd positions, for example if you
have Foo[[Category:Bar]]Baz and you delete 'ooBa' and replace it with
{image}xxx{/image}, then the category ends up inside the image.

We should always merge metadata when a segment is deleted, so that it
appears outside any new structure added.

There's a weird corner case here when a segment is removed but no
insertion is made: the removed metadata then needs to get glommed onto
the next element.  We extend the insert/replace metadata array
when this happens.

Catrope 1765e39b40 ve.dm.Transaction: Implement newFromDocumentInsertion
This function builds a transaction that takes a document slice and
inserts it back into the document it came from, applying any changes
that were made.

This makes editing document slices simple:
    slicedDoc = doc.getDocumentSlice( captionNode );
    // Edit slicedDoc using a surface
    tx = ve.dm.Transaction.newFromDocumentInsertion( doc, captionNode, slicedDoc );
    surface.change( tx );

Specifically, newFromDocumentInsertion replaces the node's contents
with the document's contents (meaning any changes made to the node in
the meantime are lost). It also merges the stores internal lists
of the two documents and remaps indexes accordingly. This means editing
of references inside of references is supported.

This functionality is not specific to slices, and can also be used to
safely insert data from a paste buffer, with internal list data being
transplanted correctly.

ve.dm.MetaLinearData:
* Make merge( [ undefined, undefined, ... ] ) return undefined rather
  than [].

ve.dm.Document:
* In getDocumentSlice, store a pointer to the original dm.Document in
  the new one, and also store the length of the internal list. This
  allows us to figure out which internal list items the two documents
  have in common when we insert the modified slice back into the main
  document.
* In getMetadataReplace, optionally take the inserted metadata as a
  parameter, to allow for operations that insert both data and metadata.
  Per Ed's review, rewrite this function to return null rather than {}
  if no metadata needs to be replaced.

ve.dm.InternalList:
* Add method to merge two internal lists

ve.dm.Transaction:
* Remove newFromNodeReplacement and replace it with newFromDocumentInsertion.
* In pushReplace, optionally take the inserted metadata as a parameter.

Roan Kattouw 393925a868 Build the internalList linmod even if it's empty
Otherwise fun exceptions occur when you have a <references /> tag
with no <ref>s, for instance.

Also disregard the internalList in the data->DOM conversion, to prevent
a nasty interaction where the whitespace information on the last element
is considered invalid because it doesn't match the internalList's.

Plus test updates from hell because this touches ve.dm.example.data

Roan Kattouw 0c16066eb3 Kill attribute order preservation
Per the bug report, it's useless because Firefox does not preserve order
in the .attributes array (but sorts it alphabetically instead), and so we
actually get the same exact behavior regardless of whether we use ordering
cleverness or just a straight-up plain object: order is preserved in Chrome,
and alphabetized in Firefox.

Ed Sanders 59bfc081df Store alien meta items' dom nodes only
We don't need to decompose and rebuild alien meta items
as they are uneditable, so just store the dom node and return that
in toDomElements.

Update test cases and several tests now need to use
assert.deepEqualWithDomElements.

Timo Tijhof b463c5d377 jshint: Fix trailing comma
Due to the "es5: true" jshint option we enabled, these
warnings were surpressed. I've disabled the option since
we no longer require it. It was enabled in 07c86fc to fix
a bug with jshint. This bug has now been fixed.

Catrope 9ff6737f4c Refactor HTML attribute preservation
Rather than using namespaced linmod attributes, store the preserved
HTML attributes in the .htmlAttributes property of the linear model
element, in a nested structure to allow for easier treatment of child
nodes. Also added attribute order preservation by storing attributes
as an object plus an array of keys.

ve.ce.Node.js:
* Remove html/* attribute synchronization. Doesn't make sense any more
  because these things aren't in the attributes object any more. I don't
  think it ever made sense because these attributes were never supposed
  to be changed anyway.

ve.ce.View.js:
* Replace renderAttributes() with a simple wrapper around
  renderHtmlAttributeList()

ve.dm.Converter.js:
* Add buildHtmlAttributeList() and renderHtmlAttributes() for building

Ed Sanders d4eef1b879 Fix range translation for surface fragments
Remove all manual changes to SF ranges as these are not
undoable. Instead change translate range to default to
outer expand and build functionality around that behaviour
never changing.

As translate range is always outer I don't think we need to
check for start and end crossing over?

Added more undo tests to assert these selections are maintained
properly, and added the test case to 'update' for when and undo
point is overwritten.

Insert content now results in a selection over the inserted
content. Most usages were expecting this anyway and were
followed up with an adjustRange(-length,0) which is no longer
necessary.

Noticed that the link inspector case was never being triggered
as word boundary was always expanding to at least one char (mainly
for Hanzi selection). This doesn't make much sense as single
spaces get auto selected so removed this functionality.

Split collapseRange out into collapseRangeToStart and
collapseRangeToEnd as this may be required to get the old
behaviour (range moves to end after insert).

Roan Kattouw e89e991037 Paragraph->heading conversion was broken when cursor next to an inline node
In this case, selectNodes() returns the paragraph, which is not a content
node, and so newFromContentBranchConversion() decides to do nothing at all.

This change makes newFromContentBranchConversion() more intelligent about
finding the content branch to operate on, fixing cases such as these
where selectNodes() returns the content branch itself rather than one of
its children.

Ed Sanders 60cf31bcb6 Allow fixUpInsertion to move offsets when inserting at start/end
fixUpInsertion now returns an object with both data and offset
which allows offset changes.

Within fixUpInsertion we lazy-generate first/lastChildStack which
is a list of parent nodes for which the current node is the first/last
child. Whenever we try to close off a node we check these stacks and
if they are populated we instead use a recursive call to start
fixUpInsertion again but with the offset shifted by 1.

Ed Sanders cd4ee24c0e Remove fixUpStack to fix blank paragraph insertion bug
The fixUpStack is actually redundant code and closingStack
and openingStack handle all our cases. It was causing the
insertion to try to correct balance itself in the middle
of inserting two paragraphs, causing the creation on an
empty paragraph between them.

Added a test case for the fix and other cases to make
sure removing fixUpStack hasn't caused problems.

Ed Sanders fdf30b1ac8 Store data in LinearData class with an index-value store for objects
Created an IndexValueStore class which can store any object and return
an integer index to its hash map.

Linear data is now stored in ve.dm.LinearData instances. Two subclasses
for element and meta data contain methods specific to those data types
(ElementLinearData and MetaLinearData).

The static methods in ve.dm.Document that inspected data at a given
offset are now instance methods of ve.dm.ElementLinearData.

AnnotationSets (which are no longer OrderedHashSets) have been moved
to /dm and also have to be instantiated with a pointer the store.

Catrope a835c03bc1 Change MetaNodes to MetaItems
Rather than meta-things being special kinds of nodes, they are now a
separate class of things (MetaItems) along with Nodes and Annotations.

* Created a generic ve.dm.MetaItem that meta items inherit from.
  There will be actual instances of this class as well in the upcoming
  meta group code.
* Renamed MetaNode to AlienMetaItem, MWMetaNode to MWMetaItem,
  'metaBlock'/'metaInline' to 'alienMeta'
* Created a MetaItemFactory, handle meta items in the ModelRegistry
* Kill ve.dm.Node.static.isMeta, now obsolete

ve.dm.Converter:
* Pass in the MetaItemFactory
* Look up data element types in the ModelRegistry rather than the
  NodeFactory, because they can be either nodes or meta items
* Document createDataElement() and make explicit that modelClass can be
  either a node or a meta item
* Handle meta items in getDataFromDom()

Catrope 1a4c60ed17 Make translateRange() translate across wrapping transactions correctly
Previously, we would translate to the right of an insertion, but for
wrapping transactions that means we end up with something like
<ul><li>|<p>...</p></li></ul>|, which doesn't make any sense. This
change changes this to <ul><li>|<p>...</p>|</li></ul>.

* Add parameter to translateOffset() that toggles the behavior for
  the offset before an insertion
* In translateRange(), translate start and end differently
** In some cases this can map them across each other, fix that

We already combine consecutive operations of other types, but didn't do
that for remove. We have to do this, though, because translateOffset()
gets confused otherwise.

Catrope 8f84982e0d Fix bug with replacements in translateOffset()
In our test case, offset 12 was mapped to 16, but that should be 11.
The problem here was that the offset right before the removal was
mapped to right after the removal, but that's only valid if we're
dealing with a removal, not when we're dealing with a replacement
where we're both removing and inserting data.

Ed Sanders 67e9d5d1dd (bug 45029) Transactions for metadata modification.
Have created builders for insertion, removal, and single element replacement.

In adding Document.getMetadata which is nearly identical to Document.getData,
the two functions have been refactored to use a common static method
getDataSlice, with this.data/this.metadata as an argument.

Document.spliceMetadata has been added. It is essentially spliceData with
the data/metadata synchronisation issue.

Metadata cursor position is now tracked in the TransactionProcessor. Cursor
advancement has been moved to a function so the metadata cursor can be reset
every time the data cursor is moved.

There were unhit bugs in the TransactionProcessor run test section, where
the data being loaded into the test documents wasn't always being deep-copied,
and the assert was looking at getData instead of getFulldata (which wouldn't
be able to test metadata changes).

Catrope 591f2e7396 Change the HTML attribute prefix from html/ to html/0/
This means that <p data-foo="bar"> will now be converted to a paragraph
with attributes {"html/0/data-foo":"bar"} rather than {"html/foo":"bar"}

This paves the way for multi-element node (about group) handling in the
node API: nodes representing multiple DOM elements will have html/i/attr
to represent an attribute of the i'th DOM element.

  JSDuck. Every class is standalone anyway. Where needed marked
  them @class + @abstract instead.

Catrope 43f1612324 No longer copy data in ve.dm.Document constructor
The data array is now taken by reference, and the caller must perform
any copying required.

Changed tests to make a deep copy of shared data sets (mostly
ve.dm.example) before passing them to ve.dm.Document().

Christian Williams f2d08f913b Added reversed boolean for translateOffset
Previously, Undo used a transaction's lengthDifference to calculate the selection to display after the transaction was undone. Now, translateOffset with the reversed boolean set to true will properly translate the inverse of a transaction's selection change. Fixes bug #40538

Catrope 5e7c14c868 Manage annotations in ve.AnnotationSet object
Introduced the ve.AnnotationSet class to manage sets of annotations. This
is a generalization of ve.OrderedHashSet, a class that manages a set
using an array and an object keyed by hash.

Converted everything that stores, tracks or passes around annotations to
use ve.AnnotationSet. In particular, this means the linear model now
contains AnnotationSets instead of hash-keyed objects.

This allows us to maintain the order of annotations in the linear model,
and will help fix bugs with annotation ordering and splitting.

Catrope 4fbabd16dd Make translateRange() map the offset immediately before an insertion to after the insertion rather than before
This will cause ve.dm.SurfaceFragment.prototype.insertContent() to place
the selection after the insertion as well.

Timo Tijhof 23c5b0d02c Make use of new jshint options
* Restricting "camelcase":
  No changes, we were passing all of these already

* Explicitly unrestricting "forin" and "plusplus"
  These are off by default in node-jshint, but some distro of jshint
  and editors that use their own wrapper around jshint instead of
  node-jshint (Eclipse?) may have different defaults. Therefor
  setting them to false explicitly. This also serves as a reminder
  for the future so we'll always know we don't pass that, in case
  we would want to change that.

* Fix order ("quotemark" before "regexp")

* Restricting "unused"
  We're not passing all of this, which is why I've set it to false
  for now. But I did put it in .jshintrc as placeholder.
  I've fixed most of them, there's some left where there is no clean
  solution.

* While at it fix a few issues:
 - Unused variables ($target, $window)
 - Bad practices (using jQuery context for find instead of creation)
 - Redundant /*global */ comments
 - Parameters that are not used and don't have documentation either
 - Lines longer than 100 chars @ 4 spaces/tab

* Note:
 - ve.ce.Surface.prototype.onChange takes two arguments but never
   uses the former. And even the second one can be null/undefined.
   Aside from that, the .change() function emits
   another event for the transaction already. Looks like this
   should be refactored a bit, two more separated events probably
   or one that is actually used better.
 - Also cleaned up a lot of comments, some of which were missing,
   others were incorrect
 - Reworked the contentChange event so we are no longer using the
   word new as an object key; expanded a complex object into multiple
   arguments being passed through the event to make it easier to work
   with and document

Trevor Parscal d8ee3c2c29 After much research on error objects, native = good, custom = bad
Stack traces, line numbers, etc. All the approaches I've seen are bad hacks. This is the best way to go.

Trevor Parscal b4de3ead08 Throw ve.Error instead of string literals
Throwing strings is bad because it doesn't include a lot of important
information that an error object does, such as a stack trace or where
the error was actually thrown from.

ve.Error inherits directly from Error. In the future we may create
more specific subclasses and/or do custom stuff.

Code conventions / Misc cleanup
 * Various JSHint warnings.

 * jQuery(): Use '<tag>' for element creation,
   use '<valid><xml/></valid>' for parsing
 * Use the default operator instead of ternary when the condition and
   first value are the same.
   x = foo ? foo : bar; -> x = foo || bar;
   Because contrary to some programming language (PHP...), in JS the
   default operator does not enforce a boolean result but returns the
   original value, hence it being called the 'default' operator, as
   opposed to the 'or' operator.
 * No need to call addClass() twice, it takes a space-separated list
   (jQuery splits by space and adds if needed)
 * Use .on( event[, selector], fn ) instead of the deprecated
   routers to it such as .bind(), .delegate() and .live().
   All these three are now built-in and fully compatible with .on()
 * Add 'XXX:' comments for suspicious code that I don't want to change
   as part of a clean up commit.
 * Remove unused variables (several var x = this; where x was not
   used anywhere, possibly from boilerplate copy/paste)
 * Follows-up Trevor's commit that converts test suites to the new
   QUnit format. Also removed the globals since we no longer use those
   any more.
