Only require event type to construct a formatter
...
Push the $wgEchoNotifications dependency to
NotificationFormatter::factory(), and only catch exceptions we're
actually expecting (NotificationFormatter::format()).
And clean up the logging to use structured logging while we're at it.

modules/.
